Priliminary Study On Human Errors Categorized by HFACS for Air Force Academy Cadets

ABSTRACT

  For Republic of China Air Force, (ROCAF), independent safety evaluation and audit systems, such as pilot reporting system, flight and base service reporting system, have established for the purpose to maintain the safety performances for years. Those open reports have educational purposes to all levels of Air Force organizations that any mishaps could be avoided if all members fully understand and learn from those reports. However, the human error is still one of the major potential threats to aviation safety, particularly, for un-experienced flight cadets. Categorized by applying human factors analysis classification system (HFACS), all the human factors have direct, indirect impacts or interactions among one another throughout 4 HAFCS levels in any flight mishaps. Through the analysis on 18 flight mishaps, this paper concludes that the preconditions for unsafe acts is the most committed errors for air force academy cadets summered from 18 flight mishaps. The findings provide primary suggestions on decision making improvements for flight training on air force academy cadets.
